The U.S. Embassy in Quito and the U.S. Consulate General in Guayaquil are pleased to announce that representatives from the Regional Federal Benefits Unit will visit Quito, Cuenca, and Guayaquil from August 29 to September 2 to offer services for beneficiaries or individuals with questions regarding U.S. Social Security and other federal benefits.
Quito
Location: U.S. Embassy Quito. E12-170 Avigiras Ave. and Ave. Eloy Alfaro

Appointments: Please schedule an appointment. To do so, send an email to FBU.Santo.Domingo@ssa.gov on the subject line write: “APPOINTMENT SSA – Ecuador”. Please include your name, Social Security number, telephone, full residency address and service you require.
You must bring your documents in original along with photocopies of each document you will submit on your request or application.
Alternatively, you may schedule an appointment by contacting the FBU Santo Domingo at 809-368-7011, Mondays and Thursdays from 8:00 AM to 11:00 AM Ecuador time.
WHAT TO BRING FOR SOCIAL SECURITY SERVICES (please bring legible copies of all documents to be submitted):
Applying for Social Security Benefits: Bring originals and copies of the following for all applicants: Birth Certificate, passport – if applying for auxiliary benefits or survivor’s benefits marriage and/or death certificates
Social Security Number Card Application: Bring a copy of your valid U.S. passport, Certificate of Birth Abroad or original birth certificate and completed form SS-5FS.
Change of Address for Social Security: Bring your current passport.
Medicare Part B Enrollment/Cancellation: To enroll in Medicare, you should complete and sign this CMS-40B and to cancel your enrollment please complete and sign this CMS-1763.
